The Mission of George Mark Children’s House is to provide an important addition to the continuum of care for children with life-limiting illnesses; enhancing the quality of life for the entire family by offering medical, emotional, spiritual, and respite support in a home-like setting that is full of life and activity regardless of ethnicity, gender identity, religion, sexual orientation, national origin, or socioeconomic status.

POSITION SUMMARY
Reporting to the Director of Nursing, the Registered Nurse will work collaboratively within the Nursing Department, and will provide direct patient care and assist in coordination of patient care services using the framework of the GMCH Mission and the philosophy of pediatric palliative care. This is an hourly, non-exempt full-time 32 hours / week position.  It includes weekend and holidays. This position may require some travel.

Responsibilities

Essential Functions:  Clinical/Technical

  •  At the direction of the patient’s primary care physician, the Medical Director and the Director of Nursing, provide and coordinate quality palliative care to patients/families, including nursing services, medication administration, treatments, diagnostic and preventative procedures.
  • In coordination with IDT (Inter-disciplinary Team) develop, implement, evaluate and update plan of care for each patient/family.
  • Assess patients and report to the managing physician reactions to treatments and medications, as well as changes in the patient’s physical or emotional condition.
  • Assess and respond to clinical and psychosocial needs of patients and families.
  • Perform nursing procedures such as blood draws, central line dressing changes, mediport access.
  • Calculate medications and fluids with 100% accuracy.
  • Assess pain and manage symptoms associated with pediatric patients requiring end-of-life care.
  • Coordinate patients’ orders.
  • Coordinate patient nursing care with other team members including other nursing staff, psychosocial staff, Pharmacist, Medical Director, attending physicians and various community agencies and resources.
  • Supervise LVNs, CNAs, and volunteers.
  • Participate in intakes, admissions, transfers and discharges.
  • Protect patients and employees by adhering to infection-control policies and protocols, medication storage procedures and controlled substance regulations.
  • Ensure proper operation of DME and other equipment by following manufacturers’ instructions; performs routine maintenance and acts as liaison with equipment providers.
  • Maintain current clinical knowledge of pediatric palliative care practices through ongoing education and attendance of in-service training sessions.

Essential Functions:  Patient Interaction

  • Actively participate in the provision of medical, psychosocial and spiritual care for GMCH children-patients and their families.
  • Meet with patients and families as needed to anticipate changes in medical, psychosocial and spiritual care needs.
  • Teach, supervise and counsel the patient and family regarding pain and symptom management.
  • Communicate with patient/family any changes in patient condition or plan of care.

Essential Functions:  Team Communication

  • Document medical interventions and maintain accurate clinical records in the Medical Record System (Netsmart).
  • Attend scheduled Inter-disciplinary Team (IDT) meetings when possible and document discussion and any changes to plan of care in the medical record.
  • Maintain communication with patient, family and hospital staff when patient is transferred and/or hospitalized.
  • Maintain patient confidence and protects GMCH operations by keeping all patient information confidential and following HIPAA at all times.
  • Check GMCH email in order to be apprised of House business and professional tasks and responsibilities.

  • Demonstrate the basic skills and core competencies of a Registered Nurse, including managing multiple, competing and varied tasks, time management, listening skills, problem solving (clinical and interpersonal), decision making (clinical and interpersonal), delegation of duties and successfully managing work-flow.
  • Perform tasks in a culturally sensitive manner without regard to any protected category as defined by federal, state or local law.
  • Provide House tours as requested.
  • Utilize GMCH resources in a cost effective manner.
  • Maintain a clean and organized work area to reduce likelihood of health and safety hazards.
  • Effectively deal with personal psychosocial and bereavement issues related to job duties.
  • Maintain professional boundaries within Scope of Practice.
  • Attend in-service training and staff meetings.
  • Adhere to all state regulations regarding professional license.
  • Report to work on time for your scheduled hours, ready for work.
  • Understand and adhere to GMCH compliance standards as they appear in the GMCH Employee Manual, GMCH Policies and Procedures Manuals , GMCH IIPP and Departmental memos.
  • Ability to communicate accurately and concisely using both written and verbal skills.
  • Ability to work independently to manage a caseload of patients.
  • Ability to access, input, and retrieve information from a computer.
  • Knowledge of office procedures and office machines.
  • Possess fluency in English.
  • Possess fluency in Spanish or other language (preferred).
  • Perform other duties, as assigned.
Qualifications

EDUCATION

  • Current California Registered Nurse License without limitations is required
  • Current BLS/CPR certification

LICENSES AND CERTIFICATION

  • Current California Registered Nurse license without limitations is required
  • Current BLS/CPR certification

EXPERIENCE

  • A minimum of 1 year RN experience required
  • A minimum of 2 years of pediatric and/or end-of-life care experience preferred

 

Job Benefits

Employees regularly working a minimum of 24 hours per week are eligible to participate in the benefits program. The cost for employees' health, dental and vision are paid in full by GMCH.
